Registering the death and the duties of the informant

To register the death the informant must be prepared to give the Registrar the following particulars relating to the deceased:

  1. The date and place of death.
  2. The full name and surname (and the maiden surname if deceased was a woman who had married).
  3. The date and place of birth.
  4. The occupation (and if the deceased was a married woman or a widow the name and occupation of her husband).
  5. The usual address.
  6. Whether the deceased was in receipt of a pension or allowance from public funds.
  7. If the deceased was married, the date of birth of the surviving widow or widower.

The deceased's medical card should be delivered to the Registrar






Persons qualified and liable to act as informants

The following persons are designated by the Births and Registration Act 1953 as qualified to give information concerning a death:-

Deaths in houses and public institutions

  1. A relative of the deceased, present at the death.
  2. A relative of the deceased, in attendance during the last illness.
  3. A relative of the deceased, residing or being in the sub-district where the death occurred.
  4. A person present at the death.
  5. The occupier* if he knew of the happening of the death.
  6. Any inmate if he knew of the happening of the death.
  7. The person causing the disposal of the body.
*"Occupier" in relation to a public institution includes the governor, keeper, master, matron, superintendent, or other chief resident officer.

Religious Services

The Officiating Minister will either visit or telephone you and may ask some of the following questions about the deceased.


  1. What name were they known by?


  2. What close family did the deceased have?


  3. Were they employed or retired - any hobbies/leisure activities?


  4. Did they have any special qualities or particular characteristics?


  5. Is there to be a mention of thanks to anyone during the service?


The Minister will endeavour to make the service personal and memorable and may ask your wishes regarding the order of service, for example:

Choice of hymns or music to be played during the service. Selected readings from the Bible, favourite poem or sentences.





Things to send back

You should return the following with a note of explanation and the date of death with each of the documents:

  • Order books, Payable orders, or Girocheques to the Social Security Office or other DSS office which issued the payment. This applies also to a child benefit book which includes payment for a child who has died. Orders should not be cashed after the death of the person. It may be useful to keep a record of pension book numbers or other Social Security numbers before you send anything back.
  • The deceased's Passport to the Passport Office. You can get the address from the Post Office.
  • The deceased's Driving Licence to DVLC, Swansea SA6 7GL.
  • The Registration Documents of a Car, for the change of ownership to be recorded.
  • A Season Ticket. claim any refund due.
  • Membership cards of clubs and associations. claim any refund due.
  • Library books and tickets.
  • Any National Insurance papers to the relevant office.
  • Any NHS Equipment such as Wheelchairs, Hearing Aids, Artificial Limbs.






People to Tell

You should tell:

  • The Local Social Services Department of the council if the person was getting meals-on-wheels, home help, or day-centre care or had an appliance or piece of equipment issued by the department.
  • Any hospital the person was attending.
  • The family Doctor to cancel any Home Nursing.
  • The Inland Revenue.
  • The Social Security Office if money was being paid directly into a Bank or Building Society Account, for example Retirement Pension, Attendance allowance.
  • Any Employer and Trade Union.
  • A child or young persons Teacher, Employer or College if a Parent, Brother, Sister, Grandparent or close friend has died.
  • A Car Insurance Company (if you are insured to drive the car under the deceased's name, you will cease to be legally insured).
  • The Local Offices of British Gas, the Local Electricity Company and the Telephone Company.
  • The Local Council Housing Department if the person who has died was living in a Council House.
  • The Local Housing Benefit/Community Charge Benefit Section if the person who has died was getting Housing Benefit and /or Community Charge Benefit.
  • The Post Office so that they can redirect the deceased person's Mail.
